What can kill a lot of long-running campaigns before they start is poor scheduling. If players aren't sure when to be there, it can be much harder to plan out a starting time for a session. It's good to make sure that everyone is clear on who's playing and when you plan to start, lest you end up with groups falling apart before the first die is rolled.
